Community Care Network Program (CCNP)

Serving as SHOPP’s unique core program, CCNP facilitates the development of a supportive network for older adults. The purpose of this program is to assess participants’ individual needs to increase access to benefits and entitlements and provide educational and recreational activities which build on individual strengths. The program also provides supportive counseling, information and referrals, and advocacy on behalf of clients to other programs and services. SHOPP Community Care Network Program services are decentralized through a network of partnerships with churches, senior apartment buildings, and private housing developments, increasing access to participants and promoting positive physical, psychological and social well-being.
